Submit copies of your proof of age, such as a birth certificate, driver’s license, or passport. If you are married, include your spouse’s proof of age and marriage certificate. Widowed applicants must provide the original death certificate of their spouse.

Proof of Age Documentation

For divorced applicants, submit all divorce decrees and settlement agreements. Disability benefit applicants must include a complete Social Security Disability Award. Always send copies, not originals, and provide the highest-preference documents available from the approved list.
